Please read the following to help in completing this form:

You have been asked to submit this form because you requested to add a third year of touring activity to your TQF multi-year agreement. 

You cannot request more than $150,000 of AQ funding for Year 3. Requests for funding that are more than 10% higher than the annual average of your previous TQF multi-year funding may not be approved unless there is a corresponding and justified increase in activity.

You cannot request funding for an industry placement.

So that we can progress your request, can you please:

• give details of your planned Year 3 activity
• update the Project Plan with the new activities
• complete the Key Outputs of year 3 of the project
• provide a budget for Year 3 of the project
• certify that the information is true and correct
• submit this form.

Arts Queensland will then assess your request to ensure it:

• is in line with the quality and impact of your original approved application
• does not include any ineligible activity
• represents good value in line with the original approved application and the scope of the new activity
• still meets the objectives of the Fund
• still meets any condition(s) of funding previously applied.

Arts Queensland will also consider your delivery against KPO targets of Year 1 and Year 2 of your project.

If your request is approved, you will receive email confirmation and a deed of variation.
